By signing below, the company identified on this enrollment form ("Company") authorizes CoreBridge, Inc., a Delaware corporation ("CoreBridge"), to withdraw any applicable one-time fee and all monthly recurring charges from the account designated above. The provisioning of the CoreBridge Management System ("System") to the Company is subject to the Company's acceptance of the CoreBridge License Agreement ("Agreement"), which will be presented to the Company for acceptance upon first logging into System. This Agreement shall be automatically renewed for an equal successive term on the anniversary date if paying annually, or on the 25th of every month if paying monthly, unless Company provides CoreBridge with notice of intent not to renew the Agreement at least 10 days prior to the end of the current term. Such notice shall be sent to billing@corebridge.net. Any applicable sales tax will be added to the total price under this Agreement. The sales tax may appear as a separate charge on your payment method. All sales are final.
